I hereby give permission for my child to participate in Sealy Soccer Factory (SSF) programs. I assume all risks in regard to the programs in which my child will participate. I release, indemnify and agree to hold harmless, SSF and its affiliated from any liability that may arise from participation in the programs organized by SSF. I understand that SSF will not distribute my details to any third parties. I hereby authorize SSF to use my child's name or pictures of him/her participating in the program for marketing purposes.
